Grand opening January 1, 2027. Enrollment opens November 1, 2026.

Enrollment

One question before you enroll.

It takes a second, and it saves us both an awkward refund conversation later.

Are you enrolled in Medicare?

This includes Original Medicare (Part A and Part B) and Medicare Advantage plans.

If you're enrolled in Medicare

We can't enroll you in the membership right now, and we'd rather tell you here than after you've paid.

Because our clinicians participate in Medicare, we aren't able to charge a membership fee for services Medicare already covers. That's a limitation on how our practice is set up. It isn't something you've done, and it isn't a judgment about your coverage.

Please still come see us.

Maxfield Medical Urgent Care is open 8am to 8pm, every day of the week, and we would genuinely like to be the people you see when something comes up — a bad cough, an injury, an infection that can't wait for Monday. You're welcome there on exactly the same terms as any other patient, and Medicare is billed the ordinary way, as it is anywhere else.

Same building, same team, and clinicians who can see your records. None of that depends on a membership.

Ask us to add you to the list

We keep a list of people who would like to join if this ever becomes possible. If it does, the people on that list are the first we'll contact.

Call us at (470) 239-4877 or email dpc@maxfieldmedical.com and we'll add your name. It takes a minute, and you'll talk to someone here rather than filling in a form.

We'll use your contact information only to reach you about membership becoming available to Medicare beneficiaries. There's no cost, no deposit, and nothing is being reserved or held. Ask us at any time and we'll take you off the list.

Maxfield Medical Direct Primary Care is a membership-based medical practice operating under a direct primary care agreement as defined by O.C.G.A. § 33-7-2.1. This agreement is not health insurance. It is not a health plan and is not a substitute for coverage. Membership does not cover hospitalization, specialist care, emergency services, or prescription medications.
